Ultravet Medical Devices – No Return Policy for Sterile, Single-Use Medical Devices

Ultravet Medical Devices (“Ultravet”) is committed to the highest standards of patient safety and regulatory compliance. To maintain the integrity of our products and to comply with applicable regulatory requirements, Ultravet maintains a strict no return policy for all sterile, single-use medical devices once shipped from our facility.

Scope

This policy applies to, but is not limited to, the following Ultravet products:

• Brockenbrough-Allen Transseptal Needles

• Ultra Steerable Guide Sheaths

• CoApt Valve

• Any other sterile, single-use device supplied by Ultravet.

Policy

1. Final Sale of Sterile, Single-Use Products

• All sterile, single-use medical devices are non-returnable and non-refundable, regardless of whether they are opened or unopened, used or unused.

2. Rationale

• Once products leave Ultravet’s chain of custody, sterility and quality assurance can no longer be guaranteed.

• Regulatory standards (including FDA and international guidelines) prohibit the resale or redistribution of such products after shipment.

3. Exceptions
   Returns will be accepted only under the following circumstances:

• Shipping Error: Products shipped in error by Ultravet (wrong item, incorrect quantity).

• Damaged in Transit: Products received with visible damage to sterile packaging.

• Product Recall or Quality Investigation: If Ultravet, at its sole discretion, requests return of product for safety or regulatory reasons.

In such cases, a Return Material Authorization (RMA) number must be obtained from Ultravet prior to any return. Unauthorized returns will not be accepted.

Acknowledgement

By purchasing products from Ultravet Medical Devices, the customer acknowledges and accepts this No Return Policy.

Limitation of Liability

Ultravet Medical Devices’ liability for any claim arising out of the sale or use of its products is strictly limited to the replacement of defective products or the refund of the original purchase price, at Ultravet’s sole discretion. Ultravet shall not be liable for any indirect, incidental, or consequential damages, including but not limited to loss of profits, arising out of or related to the use of its products.
